usermanual: Prepare to change from using numbers for chapater and section
URL's to using names. This required id's to be added to all of the sections and
chapters. I also renamed the id's in the recipes section from bb_ to
recipes_. This will allow for stable URLs into the user manual on the web
site. The change to the makefile to activate this is not yet commited.
