Modify kernel.bbclass to use KERNEL_OUTPUT consistently and default to ?=

In kernel.bbclass we don't allow for KERNEL_OUTPUT to be overriden, nor is it
consistently used.  The attached patch does the following:
- Use ?= rather than = for KERNEL_OUTPUT default case
- Change all other instances of arch/${ARCH}/boot/${KERNEL_IMAGETYPE} to
${KERNEL_OUTPUT}
